Results of a Johns Hopkins study suggest that individuals with schizophrenia are significantly more likely to live longer if they take their antipsychotic drugs on schedule, avoid extremely high doses and also regularly see a mental health professional.

Psychiatrists have long known that people with schizophrenia who stick to a drug regimen have fewer of the debilitating delusions and hallucinations that are hallmarks of this illness. But there have been concerns about whether some of the known side effects of the medications increased risk of cardiovascular disease and diabetes, for example carry higher mortality risks, the researchers say.

"We know that antipsychotic medications reduce symptoms, and our study shows that staying on reasonable, recommended doses is associated with longer life," says Bernadette A. Cullen, M.B., B.Ch., B.A.O., MRCPsych, an assistant professor of psychiatry and behavioral sciences at the Johns Hopkins University School of Medicine, and leader of the study published in Schizophrenia Bulletin. "The same is true for going to see a psychiatrist or therapist," she says, noting that regular visits to a mental health professional are one way to monitor and encourage drug-use compliance, but also in and of themselves increased survival in this vulnerable population.

Cullen and her colleagues analyzed data collected between 1994 through 2004 on 2,132 adult Maryland Medicaid beneficiaries with schizophrenia. The researchers reviewed how much medication the patients took, how regularly they took it and how often they visited a mental health professional. The goal of the study was to review how adherence to the 2009 pharmacological Schizophrenia Patient Outcomes Research Team (PORT) guidelines was associated with mortality in this population.

Comparing data from year to year, the researchers found that among those patients who had 90 percent or better compliance with their medication schedules, the risk of death was 25 percent lower compared to those who were less than 10 percent compliant. Over the decade-long study period, taking medication did not increase the risk of death and there was a trend towards reducing the mortality rate. In addition, the researchers found that each additional visit per year to a mental health professional was linked to a 5 percent reduction in risk of death overall.

Cullen's study did not rule out all links between increased mortality and antipsychotic drugs. For example, her team found that people who took high doses of first-generation antipsychotic medication daily (1500 mg or greater chlorpromazine equivalents) were 88 percent more likely to die. She says mortality rates possibly increased in this group because first-generation antipsychotics have been associated with cardiac disease risks, and among those who died while taking the larger doses, 53 percent died of cardiovascular disease. "These drugs work very well, but there is clearly a point of diminishing returns," she says. "You rarely need to be on extremely high doses."

Among those whose information was reviewed, the most common cause of death was cardiovascular disease (28 percent); unintended harm, including suicide, was responsible for 8 percent.

"If people are taking their medications, they usually have fewer symptoms and are able to be more organized in other areas of their lives," says Cullen, director of community psychiatry at The Johns Hopkins Hospital. "We believe they are then more likely to make appointments with their primary care doctors, to stay on top of other illnesses they may have and to regularly take diabetes, blood pressure or cholesterol medication that they may require to stay healthy. We also believe that they are more likely to be socially engaged and have a healthier lifestyle."

"If your illness is under control, you can do a lot more," she adds.

Cullen says the study clearly lays out the value of mental health providers to individuals with schizophrenia. Those who saw therapists or psychiatrists were more likely to survive, regardless of whether the individual also took his or her antipsychotic medication on a regular basis, she says.

This finding is crucial, she says, given that Maryland Medicaid officials are considering capping the number of mental health visits allowed each year, something the data now suggest is potentially detrimental to survival.

Cullen notes that adherence to a medication regimen and moderate first-generation antipsychotic dosing are both part of the 2009 PORT recommendations designed to guide treatment.

Tustin engineer wins small business award | program, rawal, sba ...

The owner of a Tustin engineering firm will be honored as small-business owner of the year at a luncheon Thursday as part of Minority Enterprise Development Week.

Dhananjay Rawal started Enviro Compliance Solutions Inc. in his garage in 2001 after his employer filed for bankruptcy.

"We were expecting our second child and my wife couldn't believe I wasn't going to take a job with benefits, but I needed to do it on my own," Rawal said.

The company has grown from a one-person consulting firm to nine employees doing engineering, environmental consulting and construction nationwide. The MED Week award honors owners whose businesses have participated in the U.S. Small Business Administration's 8(a) program that assists socially and economically disadvantaged business owners.

Rawal said he didn't know anything about the program until a potential customer mentioned that Enviro Compliance Solutions could get more contracts if it were certified as an 8(a) company. "I got the certification and my first contract was $160,000," Rawal said. "Now we are doing $5 million to $6 million in projects a year."

He said his company has benefited from classes in such topics as accounting principles from an SBA representative.

Rawal came to the United States from Mumbai, India, in 1988. He attended UC Irvine and then finished his education at Cal State Fullerton while he worked full time.

Also at the MED Week luncheon, Kusuma Kavia, executive vice president of Combustion Associates Inc. in Corona, will be honored as the 8(a) graduate of the year, which recognizes companies that have outgrown the government assistance program.

Marie Johns, SBA's deputy administrator, said, "The 8(a) program is one of the most effective tools we have to help small businesses and entrepreneurs in underserved communities grow and succeed."

She said the SBA has recently launched an online series of programs to help companies that are not yet ready for the 8(a) program. The first 30-minute tutorial is an overview of the program. The second 40-minute tutorial is about winning federal contracts. Two more tutorials will be released by the end of the year. The tutorials can be accessed at sba.gov/gcclassroom.

The USO and partners will recognize Veterans Day with Grant a Wish for Our Heroes, an initiative that will give Americans opportunities to thank those who serve in the Armed Forces by granting wishes for our troops and their families by supporting them in a variety of ways. Through Grant a Wish for Our Heroes, Americans can take part in supporting the unique programs and services most requested by our troops and families by supporting the USO and its many partners at uso.org/grant-a-wish.

For years, troops and families have been telling the USO what programs and services they need the most through a comprehensive annual survey, ?Tell USO,? and the USO has responded. Our programs help ease the stress of deployment for both troops and their loved ones. Others provide the comprehensive support our servicemen and women need when they return home. And many help our wounded, ill and injured troops as they recover and re-enter their new lives as veterans.

This Veterans Day, the USO asks Americans to help support these USO programs and initiatives that our troops have requested most:

  • Holiday Boxes for Deployed Troops- Many deployed troops spend holidays away from their families. The USO helps bring a touch of home to troops with boxes containing decorations games, snacks, movies and themed items. All designed to bring holiday cheer those serving in the most remote corners of the world.
  • Connecting Troops to Families Back Home- Missing the birth of your child may seem hard to imagine, but it?s a reality for many of our troops overseas. The USO has made it possible for some of our military dads to witness the birth of their child, via Skype. This November, we?ll also be helping those dads send some special items home to their newborn and mom- so they can be a part of those first precious weeks of their child?s life.
  • Support Child Heroes- Help the USO provide the book ?Little CHAMPS? (Child Heroes Attached to Military Personnel) to military families in order to ease their challenges and fears of deployment, relocation and injury of a loved one. Author Debbie Fink provides a book that tells the story of five child heroes and their military lifestyles and raises awareness of military connected challenges.
  • Education, Employment and Community Reintegration for our Wounded Warriors- USO Warrior and Family Care provides Hire Heroes USA/USO Workshops and Careers Opportunity Days for wounded, ill and injured service men and women, their spouses and caregivers. The workshops focus on resume writing, mock interviews, professional work practices and translating prior military experience into a civilian career. Career Opportunity Days are in collaboration with Hire Heroes USA and the U.S. Chamber of Commerce and offer our healing heroes, spouses and caregivers a non-traditional career fair where employers connect on a personal level based on interest and background. Employers conduct mock interviews and provide feedback, providing an additional level of support for transitioning troops to meet with employers offering jobs.
  • USO Warrior & Family Center at the Walter Reed National Military Medical Center at Bethesda- On November 7th, the USO will break ground on its second 20,000 square-foot building that will support our healing heroes and their families battling invisible wounds like traumatic brain injury and post traumatic stress. The first center at Fort Belvoir is scheduled to open in 2013 and will support our healing heroes recovering from physical wounds. The two centers will include movie theaters, classrooms, sports lounges, business centers, healing gardens and more.

Kaspersky software could secure nuclear plants

DUBAI (Reuters) - Russian anti-virus software company Kaspersky Lab is developing a secure operating system to run computers inside nuclear power plants and other vital infrastructure and industrial plants, its founder and CEO said on Tuesday.?

Eugene Kaspersky, the Russian computer scientist turned multi-millionaire cyber security expert, told reporters at a company news conference in Dubai the system was at the prototype stage, with his Moscow-based company in talks with government institutions about installing the new operating system. He declined to identify any, saying the talks were confidential.?

Energy and water plants, factories and transportation systems are typically run with SCADA (supervisory control and data acquisition) systems that are accessible via conventional computer networks, making them vulnerable to hackers, Kaspersky said.?

SCADA software is sometimes left unaltered for decades, which means that it does not get updates to protect against security bugs as they are discovered.?

"It's not possible to design SCADA in a secure way, so the most obvious solution is to have a secure envelope which monitors what's going on within SCADA," Kaspersky told Reuters after announcing the project.?

"Engineers travel with laptops, USBs, so even if the system is disconnected from the Internet there is traffic."?

Computer hacking was once seen as the preserve of rogue programmers working alone or in small groups.?

These usually targeted company websites and caused little long-term damage, but the rise of so-called "hactivist" collectives as well as suspected state-sponsored cyber attacks has shifted the threat towards government-run institutions energy, transport and telecommunications networks. Many such plants use firewalls designed to protect SCADA systems from being infiltrated by malicious bugs. But Kaspersky said he was developing an entire security-focused operating system to beef up their security further.?

Some 30,000 computers at state oil producer Saudi Aramco were infected in August, although there is no evidence that the virus got into its industrial control systems. But the Stuxnet worm virus that penetrated Iran's nuclear enrichment facility is now freely available on the Internet.?

"After Stuxnet, the attacks on Aramco and other incidents, governments and enterprises are listening," Kaspersky said.

Earlier this month U.S. lawmakers said China's top telecom equipment makers should be shut out of the U.S. market as they posed a potential security threat, but Kaspersky was unconcerned that his new operating system might attract similar concerns.?

"This system is quite compact ? it's easy to check it doesn't have extra functionality ... you own it," he added.

Travels up to 40-50%.

Education and Training:
Bachelor Degree in Business Management, Marketing or related field experience required.
Minimum five (5) to 10 years successful sales and/or sales management experience selling athlete apparel / lifestyle apparel required.
Management level experience in developing, leading and maintaining a successful sales team.
Proven track record of teamwork and working well within a matrixed organization (e.g. marketing, management, finance, R&D, customer service) to complete sales and to service customers.
Proficient with MS Office applications. SAP experience desirable.

Twilio?s CEO Jeff Lawson gave a passionate keynote today at his cloud telephony startup?s conference today peppered with big stats and quotes like ?Communications is now in the domain of software. We focus on the user, and we will win.?

Twilio now powers big names in tech like Uber, eBay and Hulu, has 150,000 developers and is expanding internationally to disrupt ye old telecom industry.

The news was unveiled today at TwilioCon, Twilio?s developer conference in San Francisco, as co-founder and CEO Jeff Lawson took to the stage to proclaim Twilio?s success in disrupting the carrier hegemony. ?In telecom, the playing field has been leveled,? he said, because of the shift away from expensive hardware to cloud-based software, such as kind that Twilio offers.

Twilio today announced that there are now 150,000 developers using its APIs, with?almost 1.5 million phone calls a day and 500 million phone calls done on the Twilio platform. [Correction: Twilio has processed 500 million total phone calls and 1.5 million phone calls per day, not 1 billion total and 1.5 million API calls as we originally published].

And Twilio is on track to accelerate with the introduction of local numbers now into 20 more countries that will take its total footprint to 40 countries. This is a significant addition, considering that in the past Twilio has ramped up in increments like this. You can see the full list of supported countries here.

The expansion means that Twilio can now offer developers local phone numbers on every inhabited content [excepting Antarctica], reaching markets where it has not been before such as Australia, parts of Asia and Latin America. (Australia, notes Lisa Weitekamp, associate product manager at Twilio, had been the most-requested country of all the ones where Twilio did not offer numbers.)

Though it has momentum, this all doesn?t mean cloud telecom is a one-horse race. Veteran telephony API provider?Voxeo Labs has 200,000 developers in its Tropo?dev community ? more than Twilio does today. There?s also?talk that Twilio has lost some key talent?around developer relations, which could mute optimism about the company.

The company is taking on carriers around the world with its innovative API-based service, which lets developers create cloud-based phone numbers in web and mobile apps that cost less than similar services offered by carriers, and are significantly easier to deploy across multiple geographies ? users do not need to strike country-by-country relationships for one thing.

But carriers are now starting to get with the program: Telefonica, for example, has its Blue Via API effort, which recently added its first outside carrier, Telenor, with plans to extend that to more in the near future. (Interesting side-note, Telefonica?s former head of developer relations for Blue Via, James Parton, now works for Twilio in Europe.) Twilio announced its biggest partnership yet with AT&T?last month, though the integration?s deployment seems to be on hold as some materials about the deal were retracted.

These partnerships are leading Twilio to start focusing not just on more universal services ? such as this 20-country expansion for voice, but also to focus more on the quality of its service. To that end, today it also announced a new Global Low Latency (GLL) and quality service created by ?employing geographically strategic data centers and intelligent routing?. In layman?s terms, this ensures that calls made on its network are only cost-effective but also that they sound good.

It also potentially introduces the idea that Twilio may at some point start offering different quality-of-service levels for users, similar to the way that carriers do.

Weitekamp notes that Twilio is starting with voice but is looking at ways of extending other services to more markets as well. SMS, she notes, needs separate mobile agreements in many countries, which will take some time to iron out. She would not comment on whether and when Twilio would roll out entirely new services such as video calls or billing.

?Striking carrier deals and deploying infrastructure around the globe was a significant undertaking,? said Thomas Schiavone, director of product management at Twilio. ?But we knew we?d be measured by our call quality and this is only way to ensure the best experience for our customers around the world.?

Twilio continues its goal to deliver one API with global reach. In July, the company announced outbound SMS to over 200 countries, and support for a variety of languages, such as Arabic, Chinese, Japanese, Greek, Russian and dozens more.

The big message of Twiliocon was the idea of prioritizing customer experience. Lawson praised Twilio developers, exclaiming ?You?re focused on the user instead of the scaffolding. Others are shackled to big hardware and big software. You?re going to win.?


Twilio offers developers a powerful API for phone services to make and receive phone calls, and send and receive text messages. Their product allows programmers to more easily integrate various communication methods into their software and programs. Twilio allows developers to use their existing web development skills, existing code, existing servers, existing databases and existing karma to solve these communication problems quickly and reliably. System is based on Asterisk.

Dash for Trash is a litter-collecting scavenger hunt in which student teams help clean up the East Side of downtown Charleston. Partnered with Keep Charleston Beautiful, our office helped create the event as a part of the national Clean Sweep initiative.?

There will be prizes given out for most points collected, most recyclables collected, and most team spirit. Breakfast and lunch will also be provided.

I had a revelation today. Not only have people become so spoiled by technology that they must always seek the best and newest of everything, and it bleeds into their love life, turning their lovers into disposables.

The major part that I have missed is Devotion. Which means to give it all to someone, and to always stand by them no matter what.

The reason relationships don't work anymore is because people have no devotion. They expect to have the perfect relationship that requires little or no effort.

The very second things get tough, its easier to leave than it is to stand by them and work through it.

If people would learn not to take for granted the loving bond they have with another person; devote themselves. Things would be different.

We have been taught to demand so much perfection, and little do we know that its what makes us imperfect is what brings us all together.

Like this site, our imperfections have brought us together.

To me, perfection is just a pretty word someone made up. It has mislead the masses and its destroying mankind slowly, more and more every day.

Perfection killed Devotion, the death of devotion killed true love.

Anyone else has any thoughts?

If you believe people are here in the world as a result of a Creator then the natural conclusion is that the Creator himself is Perfection, and as a conclusion of that he requires Devotion. But it's not a slave-to-master form of obedience, but rather to be looked at in a sense that delicious food requires to be eaten.

With that in mind, you are correct, nobody is perfect, but the entity that brought us into being is, BTW I'm not trying to soapbox, merely attempting to demonstrate not only the reasons for our existence but most importantly on-topic, the reason we even have a desire to love and be loved - could that logically be a result of random asteroid collisions?

In that same vein, a friend of mine concluded from studying the Old Testament that the context of Devotion is giving away something you highly value with the stipulation of never asking for its return. That is, people weren't necessarily all that different in the past. If they had the freedom and luxury to end relationships on a whim, they would have....but most of them didn't, so it didn't happen as much. It's not that they were more ethical/moral/family-centric, it's that they didn't have the ability they would in modern society.

I'm not saying it's right, I'm saying what you're witnessing today is what you would have witnessed 100 or 200 years ago if it was socially permitted and feasible. Although its a modern manifestation, I don't believe it's a modern attitude. In the past, there were less options and less ability to do so.

So it's not just about the time we're living in, it's a greater question of human behavior.

Personally I think you have failed to account for a very simple and essential factor: people often choose the wrong person for a relationship.

Previously, partners were carefully considered as social expectaions were somewhat high and iit was difficult to get a divorce, which encouraged people to carefully consider but also made it more difficult for those who made legitimate mistakes to seek a marriage to be absolved.
With that no longer being the case, people simply do not consider carefully enough their choice of partner.

It's something that was true before divorce was widespread and it's just as true, if not more true, now. You're just seeing it in the open much more now that the consequences have been removed (I'm not implying that consquences were good, just stating how their removal changed the dynamics).

I'm not saying it's always intentional but people often choose what they want rather than what they need, or settle for what they need and constantly wish after what they wanted, few manage to make an extremely compatible match in this regard. This factor alone probably accounts for a majority of failed relationships.

Staying together is so old fashioned nowadays.
Last night I could have gone home with a married Russian woman.
Russian couples that migrate to NZ divorce at around 75% with the first 3 years.
Kiwi guys are just more modern....

Seriously though, I've been socialising with geese this last month.
Mate for life geese.

Loyalty was an evolutionary strength for humans, now it may be a weakness.
In todays modern world....
A female can achieve better for her young.... without the assistance of a male

Especially males with skid mark undies, sweaty balls, snorers, or mouth breathers.

Plus the economy performs better when the house gets sold, lawyers get paid, dating sites get money, hairdressers, fashion, sex aid pills, gym memberships................

follow the money.........

staying together is not good for the economy ---- short term

but thats how the thinking is nowadays, short term gains only

BTW In New Zealand divorce law changed in 1975 allowing half of the property and assets to go to both the man or woman. In the following 6 months, my parents and 2 best friends parents all divorced.

I think lawyers were real busy that year....

Before that time the woman walked away with nearly nothing.....

But usually stayed and suffered their unfair state sanctioned husbands domination

The law here changed in 1975 too and the divorce rate skyrocketed since there was such a backlog of people who were previously unable to get divorced.

DWH: I agree that it is better to be able to get a divorce.
But there are consequences to both being able to and being unable to, which I was simply commenting on in my previous post out of fact rather than opinion.
