David A. Margileth, M.D.:?There are two main pathologic types of breast cancer.? With invasive breast cancer, 80% of breast cancer is what is called invasive ductal and about 10% is invasive lobular.
Lobular tumors are characterized by a couple of difficulties.? Number one, they are harder to diagnose by mammogram as their consistency is often the same as normal breast tissue and therefore don?t show up as well on mammogram and may be diagnosed later than a comparable ductal carcinoma.
The other dilemma with lobular carcinoma compared to ductal carcinoma is that they tend to be fairly chemotherapy resistant.? However they also tend to be much more sensitive to hormone manipulation and therefore in general with a lobular carcinoma either in the adjuvant setting or the metastatic setting one has a strong preference for trying to use various antiestrogen therapies and delaying chemotherapy as long as possible.? Those antiestrogen therapies may include tamoxifen, the three aromatase inhibitors and also an intermuscular injection called Faslodex.? So in general with lobular tumors one has a preference for using hormone therapy rather than chemotherapy.
